This accessibility statement applies to the Flu Survey web site at https://flusurvey.net/.
This website is run by the UK Health Security Agency (UKHSA). We want as many people as possible to be able to use this website. For example, that means you should be able to:

Non-compliance with the accessibility regulations
The images to select a profile image lack an accessible name. This fails WCAG 2.1 success criterion 1.1.1 (Non-Text Content).
Some radio buttons within the survey lack an accessible name. This fails WCAG 2.1 success criterion 1.1.1 (Non-Text Content).
Some tables are used for layout purposes but are not marked up as being used for presentation. This fails WCAG 2.1 success criterion 1.3.1 (Info and Relationships).
Within the survey, the questions are not grouped with the form inputs. This may result in the form inputs lacking context when focused by a screen reader. This fails WCAG 2.1 success criterion 1.3.1 (Info and Relationships).
Some pages may not have a correctly structured heading hierarchy, with some pages missing an H1 element or skipped heading levels being present. This fails WCAG 2.1 success criterion 2.4.1 (Info and Relationships).
Some pages may not have a title that represents the page content. This fails WCAG 2.1 success criterion 2.4.2 (Page Titles).
Some form fields do not convey that they are required fields to users of screen readers. This fails WCAG 2.1 success criterion 3.3.2 (Labels or Instructions).
Some error messages do not provide suggestions to correct the error present on the page. This fails WCAG 2.1 success criterion 3.3.3 (Error Suggestion).
Users of Dragon Naturally Speaking were unable to interact with the slider element as part of the survey using any known voice commands. This fails WCAG 2.1 success criterion 4.1.2 (Name, Role, Value).
Disproportionate burden
Navigation and accessing information
There's no way to skip the repeated content in the page header (for example, a 'skip to main content' option).
It's not always possible to change the device orientation from horizontal to vertical without making it more difficult to view the content.
It's not possible for users to change text size without some of the content overlapping.
Interactive tools and transactions
Some of our interactive forms are difficult to navigate using a keyboard. For example, because some form controls are missing a 'label' tag.
We aim to review the possibilities and resources needed to address the issues with navigation and accessing information, and with interactive tools and transactions. We believe that doing so now would be a disproportionate burden within the meaning of the accessibility regulations. We will make another assessment when the supplier contract is up for renewal.
What we're doing to improve accessibility
We are working alongside other government departments and agencies to fix content which fails to meet the Web Content Accessibility Guidelines version 2.1 AA standard. Also we are aware of the new requirement for WCAG 2.2 and we will work on achieving that.

This website was last tested on 23rd August 2023. The test was carried out by Zoonou who used WCAG-EM to define the pages tested and test approach.
